    Hi all
    60 yo Healthy woman, no meds
    One month ago "mild" trauma Left face and left eye
    Left eye was swollen for a few days; resolved
    No clear concussion sequelae.
    Experiences a visual Kaleidoscope for  one minute at a time
    (at most 2 minutes)
    Entire field of vision ; can still see , "But I couldnt Drive like that"
    No Hx Migraine in patient or any family members
    4 episodes in the last month
    First two episodes were monocular  , Left eye
    Last two episodes , both eyes.
    Vision is 20/20 both eyes.
    Neurophthalmic and Retinovascular exam is normal
    Autoflourescence is normal.
    No enlarged Blind spot.
    CTA Neck and Brain Normal
    MRI Brain Normal.
    Considering  EEG/ERG if continues....

    This could be trauma induced migrain but it should be with head trauma , a trauma that cause eye swelling is not that Mild, the exact type and circumstances of trauma may explain

    Very interesting discussions, every case gives a new insight, a new opportunity to learn, thanks for sharing

    EEG may be yes, but an ERG in episodic visual phenomena may not help.

    Once the changes are noticeable on the ERG, logically, the symptoms should be present all the time.

    Hi,

    Can you get a more detailed description of the kaleidoscope? Was it geometric shapes or blobs of color? The former suggests migraine, the latter suggests seizure. I suspect that the uniocular event was really hemianopic if she couldn't have driven during it.

    Although the International Classification of Headache Disorders indicates that aura needs to last at least 5 minutes, it can be shorter.
