JNO now requires any publication, including UPFRONT, that uses patient-based materials, to obtain a patient consent to use these materials in a publication. Most institutions are now including such permission in their institutional "sign-in" forms. These would suffice. If your institution does not do that, you can ask if they have such a form, otherwise, you can ask JNO for its form. You can retain it, but you will be asked to confirm that you have such permission.
