Connecticut neuro-ophthalmology/oto-neurology referral request

  • 1.  Connecticut neuro-ophthalmology/oto-neurology referral request

    Posted 2 days ago

    Dear NANOS community,

I am seeking help identifying a neuro-ophthalmologist in CT, ideally with some oto-neurology/vestibular neurology expertise, for a patient who is a friend of a friend. She is a young woman in her mid- to late 30s with hx of migraines, retinal detachments, episodic photopsias OU, positional dizziness, and recent MRI brain showing >8 mm cerebellar tonsillar ectopia sans syrinx on spine MRI. BMI >25. She is seeking evaluation by a neuro-ophthalmologist. Any recommendations would be greatly appreciated.

    Best,
    KG

Dr. Forman, thank you so much for your kind reply and willingness to help. What would be the best way for her to get connected with your clinic in Belchertown?

It sounds like she has a spinal CSF leak!

And check for Ehlers Danlos or Marfans if there is a confirmed leak. And an MRI spine and brain with contrast could help confirm leak.

MRI brain needs to be done with contrast (sounds like she at least already had a contrast scan – can you post the midline sagittal T1-weighted image?)

Spine MRI – specify looking for a CSF leak, needs STIR (heavily weighted T2, fat suppressed) images and a neuroradiologist who is familiar with looking for leaks.

Drs. Friedman and Singman, thank you both very much for your thoughtful input and recommendations. Just to clarify, I am not directly involved in her care and have not personally reviewed her imaging or medical records; this was simply a quick phone consult while trying to help connect her with a neuro-ophthalmologist closer to her in Connecticut.
