  • 1.  How to order "rule out" labs

    Hello, first year resident here (not really)...

    When ordering a test for something, for example myasthenia gravis, the lab requires a diagnosis. When in a situation where the test is not definitive, i.e. you know they have it but you're verifying, you put down "Myasthenia Gravis" and leave it at that. When you're seeing someone who might have the diagnosis you're testing for it, it's been said that you should not, indeed can not, put anything resembling "rule out myasthenia gravis".

    How are you all dealing with this dilemma?

    use the symptom that leads you to suspect that diagnosis such as ptosis or diplopia

    Maybe it’s a regional thing, but they won’t accept that.

    I use the actual diagnosis. This is no dilemma to me. Insurance issues have little to do with real medicine that we do "on the ground".

    I do Myasthenia Gravis and I add a little comment on Dx:  "Myasthenia Gravis-Suspect"
