I was wondering if anyone has further ideas?:
64-year-old man
History of lef-sided occipital pilocytic astrocytoma, which was resected in October 2024.
His main complaint is a feeling of pressure in his left eye.
He also mentions seeing light flashes that only appear in darkness upon head movement and only in the left eye.
Both complaints started in August 2024 and have remained constant, i.e. not more pronounced in nature, nor more frequently.
Eye movement does not provoke light flashes. His right eye feels normal to him.
Sudden, loud sounds do not oprovoke light flashes.
Percussion on the sinuses is symmetric and not painful.
Corneal sensitivity is symmetric.
There has already been some work-up (normal or negative):
- Carotid ultrasound
- CT-angio carotids
- MRI brain
- FF-ERG
- Humphrey automated perimetry
- OCT
- Neurological advice (no epilepsy on EEG; trial with anti-epileptics negative)
- ENT advice
I asked for an additional MRI orbits, but would welcome any further inspiration...
